can apply for correction in the death certificate. Now we discuss the detailed
procedure for correction in the death certificate.
DOCUMENTS REQUIRED:-
- An application to District/Local Registrar Birth & Deaths.
- Self Declaration by the applicant along with photograph and cross
signature.
- Certificate by 02 Witnesses. Sarpanch/Numberdar/Panch in case of village & 02 Municipal Councilors in case of the city.
- Pass Book of Bank/Post Office and I.D Card in case of a government job.
- Report of cremation by a funeral home in case of deceased expired in the city.
- Report regarding ashes flowed in holly water.
- Police report in case of accidental death.
- Any Will, Property papers, post mortem report, hospital record of
deceased.
- In case of an application for correction then clarify the same.

attested copies. Kindly write your mobile number on file and complete your file
properly so that it can be processed for further procedure.
WHERE WE CAN
APPLY FOR DEATH CERTIFICATE:-
District Registrar of Birth & Deaths authorize for rural area
entries and Local Registrar of Birth & Deaths is authorized for local city
area entries. Now we can only submit our file through Seva Kendra.
WHO CAN APPLY:-
Any family member can apply for late entry of death.
