• Self Declaration by the applicant along with photograph and cross signature.
• Certificate by 02 Witnesses. Sarpanch/Numberdar/Panch in case of village & 02 Municipal Councilors in case of the city.
• Pass Book of Bank/Post Office and I.D Card in case of a government job.
• Report of cremation by a funeral home in case of deceased expired in the city.
• Report regarding ashes flowed in holly water.
• Police report in case of accidental death.
• Any Will, Property papers, post mortem report, hospital record of deceased.
• In case of an application for correction then clarify the same.
